New Hire Forms Completion Process
This simple Web page will streamline your on-boarding process with Innovex. Just follow the directions at the beginning of each section. Return all completed and signed documents within two business days in the FedEx envelope that was included in the package with your offer letter.
Use this checklist to ensure you have completed all necessary forms.
Please note that some forms request your employee identification number or Quintiles identification number (QID#) (they are one in the same). If your manager is unable to provide you with an employee ID number, please leave the field blank and proceed with mailing within 2 business days of receipt. The HR Professional Services Center will take care of completing the blank fields once the employee identification number is issued.
Section I
Please read the Employee Guide and PhRMA code. Once you've read them, please print and sign the acknowledgement form (signature page) in the last page of each document.
You will need to mail the signed acknowledgement forms along with other completed forms in this section within two business days in the DHL envelope provided unless otherwise instructed.

- PhRMA Code on Interactions with Healthcare Professionals
- Proof of Automobile Insurance Declaration Page (photocopy) for employees receiving a car allowance. This must reflect the required amounts of coverage of $100.000/person: $300,000/occurrence: $50,000/property damage. Note: Employees must be named as the insured driver and place of residence must also be included on the Declaration Page.

Section II
Please print and complete Section 1 of the I-9 document. There is an example of an I-9 for your reference. You will need to bring the I-9 to a Notary along with one or more of the eligible identification documents mentioned on page 5 of the I-9 form. Only valid identification documents can be accepted, no expired documents will be eligible.
Please follow the Notary instructions attached.

Section IV
You have been provided with the choice of up to two Drug Screen collection sites nearest to your residence. You must visit one of these sites listed with the chain of custody form provided. If one of the sites listed is not within a reasonable distance, please call Customer Service at the Quintiles North American Professional Services Center at (800) 526-7094. The drug screen must be taken within 48 hours of receiving this notification.
